Survey of Needs for DD Waiting List  

The Health Reform Legislative Task Force is committed to providing Arkansans with the best possible Medicaid services, including assistance for those who are presently on the DDS Waiver Wait list.  In order for the Task Force to better understand community and home service needs and to determine the funding necessary to obtain these services, feedback from those who are affected would be beneficial.      

Because you or someone in your household has applied for services or is currently on the DDS Waiver Wait list, we ask for your participation in a short survey.  To complete the survey, please open an Internet browser and go to the following URL:

If you prefer, you may send an email to [email protected] and a link to this URL will be sent immediately by reply email or call 501-683-0569 and we will enter it directly on your behalf.  If you do not have Internet access, please visit your county office or local library for assistance.  We ask that you complete the survey once for each person in your household who is waitlisted for DDS services.  

The survey link for capturing data will be open for two weeks beginning March 15, 2016.  

Please understand that completing the survey is not a guarantee of services or additional funding.  Your input, however, will provide valuable information which could potentially influence the Legislative process.
